Sign Up for the Birthday Book Club!

Horizon's Birthday Book Club is a fun way to celebrate your child while supporting our school library! By joining the Birthday Book Club, your child will be invited to a special birthday celebration held during the school day with other Birthday Book Club members. They will enjoy a delicious treat from Jeremiah's Italian Ice, choose a brand-new book to keep as their very own, and leave with other special birthday prizes. Birthday celebrations are grouped throughout the year based on students' birth months, allowing us to celebrate everyone together. Best of all, your membership helps us purchase new books and resources for all Horizon students to enjoy. It's a wonderful way to honor your child while helping foster a love of reading across our entire school community!
Membership is $25. To sign up, simply complete the form (check your child's take-home folder) and submit it along with your $25 payment. I have also attached the form below if the paper copy gets lost in the shuffle! A digital registration form is also available HERE or on the Library Media Center page of our school website. We accept cash, check (payable to Horizon Elementary School; memo: Birthday Book Club), or online payment through PowerSchool or by clicking the Online Payments button on our school website. If paying by check, please include your driver's license number and two phone numbers. Both the registration form and payment must be received by Friday, August 28, for students in all birthday groups.
